Session Abstract – PMWC 2017 Silicon Valley
Session Synopsis: Precision medicine is empowered by access to diverse data, patients, and provider networks. This session will describe the role of health systems as translational research partners that can drive: 1) molecular discoveries; 2) provider and patient engagement through an interoperable platform; and 3) rapid translation of discoveries to clinical practice.

Precision Care for MS: Making It Work
Precision Care is a Sutter R&D initiative to ensure patients receive the right care at the right time during ambulatory encounters. We’ve developed a digital platform that integrates applications that integrates in real time with EHRs to obtain PRO, automates visualizing an integrated display of patient data, and supports shared decision making.
